Job summary

Ascend Hospice & Palliative Care NJ is seeking a Social Worker to support patients and families facing terminal illness. The role includes psychosocial assessments, counseling, and crisis intervention to promote comfort and safety.

The ideal candidate holds a Master’s in Social Work with at least one year of medical social work experience; hospice exposure is preferred. Travel may be required; strong communication skills are essential to coordinate care with families and providers.

Qualifications

• Master's Degree in Social Work required.
• Minimum 1 year Medical Social Work experience, hospice preferred.
• Must have Licensed Social Worker (LSW) certificate.
• Understanding of hospice philosophy and end-of-life care.
• Excellent verbal and written communication skills.

Responsibilities

• Assess emotional factors related to terminal illness.
• Assess environmental resources and safety obstacles.
• Provide emotional support, crisis intervention, and counseling.
• Coordinate with community agencies and referrals.
• Evaluate patient/family response to psychosocial interventions.
• Address cultural diversity needs and family roles.
• Perform initial psychosocial assessments and assess suicide/abuse risk.
