Job summary

Ascend Hospice in Virginia seeks a dedicated Social Worker to assess emotional factors in terminal illness, provide emotional support, and conduct crisis intervention and family counseling.

The role includes initial psychosocial assessments, coordinating with community agencies, and evaluating response to interventions, with travel to care sites as required. A Master’s in Social Work and LSW licensure are required.

Qualifications

• Master's in Social Work is required; state licensure (LSW) is mandatory.
• Minimum 1 year of medical social work; hospice experience preferred.
• Strong verbal and written communication skills are essential.
• Willingness to travel to patient locations as needed.

Responsibilities

• Assess emotional factors related to terminal illness and provide support.
• Conduct psychosocial assessments and crisis intervention as needed.
• Coordinate with community agencies for referrals and interagency cooperation.
• Monitor patient/family response to psychosocial interventions.

• Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals

• Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
• Performs comprehensive initial psychosocial assessments Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect

Education:

Master's Degree in Social Work

Professional Experience:

• Minimum 1 year of Medical Social Work experience, Hospice preferred
• Must have a Licensed Social Worker (LSW) certificate (remove if not applicable)
• Understanding of hospice philosophy related to families in crisis and end of life issues
• Ability to multitask & prioritize work load
• Must be willing to travel
