Job summary

Community Healthcare of Texas in Fort Worth is seeking a Social Worker to provide therapeutic and emotional support to patients and families, educate on end-of-life issues, and connect them with useful community resources.

You will advocate for patients' needs with the care team, contribute to the Plan of Care with the patient and family, and ensure daily documentation is completed in accordance with policy.

Qualifications

• Master's degree in Social Work required.
• Hospice or healthcare experience preferred.
• Proficient computer skills required.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide therapeutic counseling and education on end-of-life issues.
• Contribute to and execute the Plan of Care with the patient and family.
• Record daily documentation of visits and contacts per policy.
• Provide referrals to community resources.
• Perform other duties as assigned.
