ACG Hospice is hiring a Registered Nurse in Missouri to evaluate patients, develop individualized care plans and coordinate with families and the care team. You will lead the care team to ensure compassionate, high-quality patient experiences on a daily basis.

Qualifications

• RN license and active state licensure.
• Graduate of accredited nursing program.
• At least 2 years nursing experience; hospice experience preferred.
• Must have reliable transportation and ability to lift 80–100 lbs.

Responsibilities

• Evaluate patients and create care plans.
• Communicate with patient, family and care team.
• Serve as driver of care team to ensure quality care.
