Job summary

The Social Worker collaborates with the care team, completes assessments within 5 days of admission, and ensures documentation and confidentiality.

The role requires an MSW (CSWE accredited) or equivalent with supervised experience, licensure as required, and prior health-care social work experience.

Qualifications

• Master’s degree from a school of social work (MSW) accredited by the CSWE; or a baccalaureate in social work with supervision by an MSW.
• Has a baccalaureate degree in psychology, sociology, or related field and supervised by an MSW.
• Must be licensed in the state where care is delivered (if required).
• At least one year of social work experience in a health care setting.

Responsibilities

• Assists the care team in understanding social and emotional factors related to health problems.
• Assesses social and emotional factors within 5 days of admission to estimate capacity to cope.
• Helps patients and families understand and follow medical recommendations.
• Utilizes resources to assist the patient and family within illness limitations.
• Observes, records, and reports social and emotional changes; documents services.
• Participates in the Interdisciplinary team and care plan development.
