Job summary

Solara Hospice in Eastern Massachusetts is seeking a compassionate Hospice Social Worker to provide emotional, psychosocial, and practical support to patients and families. You will collaborate with an interdisciplinary team to ensure holistic, patient-centered care.

You will conduct psychosocial assessments, counsel families, assist with advance directives and care planning, coordinate community resources, and maintain compliant documentation.

Qualifications

• Licensed Social Worker in MA (LCSW or LMSW) required.
• Hospice or healthcare experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Provide counseling and emotional support to patients and families.
• Assist with advance directives and care planning.
• Coordinate community resources and support services.
• Participate in interdisciplinary team (IDT) meetings.
• Maintain accurate and timely documentation.
• Ensure compliance with hospice regulations and standards.
