Job summary

St. Croix Hospice is seeking a compassionate Social Worker to support patients and families within the care team. You will conduct psychosocial assessments, provide counseling and bereavement support, and coordinate with community resources to develop care plans aligned with each patient’s goals.

Strong licensure in the state, HIPAA compliance, and collaboration with a multidisciplinary team are essential. The role follows a M-F 8:00am–5:00pm schedule with full-time benefits included.

Qualifications

• Bachelor’s degree in social work from CSWE-accredited program.
• Current Social Work license in practicing state(s).
• Three licensure levels: LBSW, LSW, LCSW.
• Ability to pass background, consumer report, and drug test.
• Provide counseling, crisis intervention, and bereavement support.
• Collaborate with interdisciplinary team and ensure HIPAA compliance.

Responsibilities

• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.
