Job summary

Crown Hospice in Victoria, TX is seeking a Licensed Hospice Social Worker (LBSW, LMSW, or LCSW) to join our compassionate interdisciplinary team. You’ll provide emotional support to patients and families navigating end-of-life care and help develop individualized psychosocial plans.

Required: MSW from an accredited program, Texas MSW license, 1 year healthcare experience (hospice preferred), strong communication, CPR, and a valid driver’s license. This full-time role offers a competitive salary.

Qualifications

• MSW from an accredited program is required.
• Current Texas Social Work license (MSW) is required.
• Minimum of one year of healthcare experience (hospice or home health preferred).
• Strong communication, organization, and relationship-building skills.
• Current CPR certification or willingness to obtain.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide emotional support and counseling to patients and families navigating end-of-life care.
• Complete psychosocial assessments and develop individualized plans.
• Connect patients and families with community resources and supportive services.
• Advocate, educate, and guide families throughout the hospice journey.
• Collaborate with nurses, physicians, chaplains, aides, and other team members to provide holistic care.
