Qualifications

• Master’s Degree from a CSWE-accredited school of Social Work.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Licensed Social Worker in the state of residence (preferred).
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Assists patient, family, physician and care team to understand social factors affecting health.
• Contributes to a comprehensive, integrated Plan of Care on a daily basis.
• Instructs care team on available community resources as needed.
