Job summary

UnitedHealth Group's Heart 'n Home Hospice is seeking a Social Worker to provide medical social services under physician direction and an interdisciplinary team. The role focuses on understanding social and emotional factors impacting health and helping patients cope, with emphasis on coordinating a Plan of Care.

The position offers a $7500 sign-on bonus for external candidates and a comprehensive benefits package.

Qualifications

• Master's Degree from a CSWE-accredited school of Social Work.
• Current Driver's License and access to dependable transportation.
• OR: Licensed Social Worker in Oregon.
• 1+ years of social work experience in a health care setting (preferred).
• Current CPR certification or the ability to complete within 90 days of hire.
• Licensed Social Worker in the state of residence (preferred).

Responsibilities

• Assists the patient, significant others, physician and health care team to understand significant personal, emotional, environmental and social factors related to the patient's health status.
• Contributes to the development of an integrated Plan of Care for patients on a daily basis.
• Instructs health care team on community resources to assist patients when needed.
