Job summary

Heart 'n Home Hospice, part of the Optum family, seeks a Social Worker in Hermiston, Oregon, to deliver medical social services under physician direction within an interdisciplinary team.

Required: MSW, OR Social Worker license, CPR certification; Driver's license and reliable transportation. Salary ranges from $63,080 to $94,620 annually for full-time staff, with a $7,500 sign-on bonus for external candidates.

Qualifications

• Master's Degree from an accredited School of Social Work
• Current Driver's License, vehicle insurance, and access to dependable transportation
• Licensed Social Worker in the state of residence (OR)

Responsibilities

• Assist the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contribute as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instruct health care team members on community resources available to assist patients on a as needed basis
