Spring Valley Hospice in Tupelo, MS is seeking a dedicated Hospice RN Case Manager to provide patient-centered care in a supportive team setting. You will assess needs, develop and implement personalized care plans, and deliver direct nursing care including medications and interventions.

Qualifications

• Graduate of an accredited nursing program with active RN licensure.
• 1 year nursing experience minimum; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assess, monitor, and address physical, emotional, and psychosocial needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team members.
• Maintain accurate and timely documentation and participate in on-call rotation.
