Spring Valley Hospice is seeking a dedicated Social Worker in Olive Branch, MS, to provide emotional support, counseling, and resource coordination for patients facing terminal illness. The role emphasizes holistic care, dignity, and respect, with a supportive team to back independent work.

Ideal candidates hold CSWE-accredited MSW or BSW, have verifiable licensure, and strong communication and organizational skills; hospice experience is preferred.

Qualifications

• MSW accredited by the Council on Social Work Education.
• Bachelor’s degree in social work accredited by the CSWE.
• Minimum of one year experience in health care; hospice experience preferred.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with insured automobile.
• Demonstrates good verbal and written communication, and organizational skills.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans when needed.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team to support holistic patient care.
• Support funeral, financial, and legal planning with families.

Our Social Worker s provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect . Qualifications

• A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in
• 418.114(b)(3).
• Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
