Job summary

Spring Valley Hospice in Olive Branch, MS is seeking a compassionate Social Worker to join our team. The role focuses on assessing psychosocial needs, providing counseling, and coordinating resources to support patients and families facing terminal illness.

You will work within a team to deliver holistic care, educate families on advance directives, and help plan for funeral, financial, and legal needs. A CSWE-accredited MSW or BS in Social Work is required, with hospice experience preferred.

Qualifications

• Master’s in social work accredited by CSWE.
• Minimum 1 year health care/hospice experience preferred.
• Current licensure to practice in state of operation.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.

Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. Qualifications

• A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).

• Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.

• Experience in a hospice care preferred.

• Demonstrates good verbal and written communication, and organization skills.

• Current licensure according to Medicare guidelines to practice in state of operation.

• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
