Job summary

The Pennant Group in Gillette, Wyoming is seeking a Medical Social Worker to provide psychosocial assessments and support to patients facing terminal illness within the hospice program.

The role requires a CSWE-accredited BSW, CPR certification, valid driver’s license, and excellent verbal/written communication; hospice experience preferred. This part-time position offers flexible hours and the opportunity to collaborate with an interdisciplinary team at Symbii Home Health.

Qualifications

• A bachelor’s program in social work accredited by the CSWE is required.
• Preferred: one year in health care; hospice experience preferred.
• Strong verbal and written communication and organizational skills.
• Must have CPR certification and a valid driver's license with access to a vehicle.

Responsibilities

• Assess psychosocial status of patients and families/caregivers related to terminal illness and communicate findings to the care team.
• Conduct social evaluations and plan interventions; counsel patients and families as needed.
• Maintain clinical records for all patients referred to social work.
• Provide information and referrals to patients, families, and community agencies.
• Serve as liaison between patients/families and community resources; participate in care planning and meetings.
• Support quality improvement activities and other duties as delegated by supervisor.
