Job summary

Ascend Hospice & Palliative Care NJ is hiring a Social Worker to assess emotional factors related to terminal illness and provide counseling to patients and families. The role involves coordinating with community resources, evaluating psychosocial responses, and addressing cultural considerations in end-of-life care.

Qualified candidates have a Master’s in Social Work and a Licensed Social Worker (LSW) certificate, with at least one year of medical social work experience, hospice preferred.

Qualifications

• Master's degree in Social Work is required.
• Licensed Social Worker (LSW) certificate is required.
• Minimum 1 year of medical social work experience; hospice preferred.
• Understanding of hospice philosophy and end-of-life care.

Responsibilities

• Assess emotional factors related to terminal illness.
• Assess environmental resources and safety obstacles.
• Provide emotional support, crisis intervention, and individual/family counseling as needed.
• Liaise with community agencies to coordinate referrals and services.
• Evaluate patient/family response to psychosocial interventions.
• Consider cultural diversity in communication and caregiving needs.
• Perform initial psychosocial assessments and assess risk for suicide/abuse/neglect.

The Social Worker will be responsible for but is not limited to:

1. Assesses emotional factors related to terminal illness.
2. Assesses environmental resources and obstacles to maintain safety
3. Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
4. Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
5. Evaluates patient/family response to psychosocial interventions
6. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
7. Performs comprehensive initial psychosocial assessments Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect

Education:
