Job summary

Ascend Hospice is seeking a dedicated Social Worker to support patients and families facing terminal illness with compassion and expertise. You will assess emotional factors, safety, and psychosocial needs, and provide counseling and crisis intervention when indicated.

Join a collaborative team committed to quality end-of-life care. A Master’s in Social Work and an active LSW are required; travel may be required; hospice experience is preferred and valued by our compassionate culture.

Qualifications

• Master's degree in Social Work required.
• Minimum 1 year of medical social work experience; hospice preferred.
• Licensed Social Worker (LSW) certificate required.
• Willingness to travel and strong communication skills.

Responsibilities

• Assesses emotional factors related to terminal illness.
• Assesses environmental resources and obstacles to maintain safety
• Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
• Maintains contact with community agencies and services to promote interagency cooperation and referrals
• Evaluates patient/family response to psychosocial interventions
• Assesses cultural diversity needs including communication, space, role of family members and traditions
• Performs comprehensive initial psychosocial assessments; assesses risk of suicide/abuse or neglect

TheSocial Workerwill be responsible for but is not limited to:

1. Assesses emotional factors related to terminal illness.
2. Assesses environmental resources and obstacles to maintain safety
3. Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
4. Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
5. Evaluates patient/family response to psychosocial interventions
6. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
7. Performs comprehensive initial psychosocial assessments Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect

Education:

Master's Degree in Social Work
