Ascend Hospice & Palliative Care MO in Lee's Summit, MO is seeking a dedicated Social Worker to join our hospice team. The role focuses on psychosocial assessment, counseling, and coordinating care for patients and families facing end-of-life challenges.

Responsibilities include emotional support, crisis intervention, and collaboration with community resources. A Master’s in Social Work, 1 year hospice-related experience, an active LSW license, and willingness to travel are required.

Formación

• Master's degree in Social Work required.
• Minimum 1 year of Medical Social Work experience, Hospice preferred.
• Must have an LSW certificate.
• Understanding of hospice philosophy for families in crisis and end of life issues.
• Ability to multitask and prioritize workload.
• Must be willing to travel.
• Strong verbal and written communication skills.

Now Hiring – Social Worker – Lee's Summit, MO

Hours: M-F 8:30-5

TheSocial Workerwill be responsible for but is not limited to:

1. Assesses emotional factors related to terminal illness.
2. Assesses environmental resources and obstacles to maintain safety
3. Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
4. Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
5. Evaluates patient/family response to psychosocial interventions
6. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
7. Performs comprehensive initial psychosocial assessments Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect

Education:

Master's Degree in Social Work required

Professional Experience:

• Minimum 1 year of Medical Social Work experience, Hospice preferred
• Must have a Licensed Social Worker (LSW) certificate
• Understanding of hospice philosophy related to families in crisis and end of life issues
• Ability to multitask & prioritize work load
• Must be willing to travel
• Strong verbal and written communication skills.
