Job summary

Ascend Hospice is seeking a Social Worker to assess emotional and environmental factors impacting terminally ill patients and to provide counseling and crisis intervention. The role supports interagency referrals, evaluates outcomes, and addresses cultural needs.

Travel may be required to serve patients and families with compassion and professional integrity. The team focuses on delivering dignified care, collaborating with caregivers, and maintaining high standards of psychosocial support for

Qualifications

• Master's Degree in Social Work required.
• Minimum 1 year of Medical Social Work experience; hospice preferred.
• Licensed Social Worker (LSW) certification required (remove if not applicable).
• Knowledge of hospice philosophy related to families in crisis and end-of-life issues.
• Ability to multitask and prioritize workload; travel willingness.
• Strong verbal and written communication skills.

Now Hiring –Social Worker

TheSocial Workerwill be responsible for but is not limited to:

1. Assesses emotional factors related to terminal illness.
2. Assesses environmental resources and obstacles to maintain safety
3. Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
4. Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
5. Evaluates patient/family response to psychosocial interventions
6. Assess special needs related to cultural diversity including communication, space, role of family members and special traditions
7. Performs comprehensive initial psychosocial assessments Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect
