The Licensed Social Worker is responsible for implementing standards of care for medical social work services and ensuring adherence to all conditions in the Service or Employment Agreement.

Essential Job Functions/Responsibilities

1. Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment, communicating findings to the interdisciplinary care team. Provide assessments at the patient's residence and assist with alternative plans if safety is an issue.
2. Conduct social evaluations including family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, or abuse, and plan interventions accordingly.
3. Counsel patients and families/caregivers on stress and coping difficulties; provide crisis intervention when necessary.
4. Assess cultural needs of patients and families, educating the care team on communication, family roles, traditions, and cultural considerations.
5. Work under supervision of a Masters Prepared Social Worker (MSW).
6. Maintain clinical records for all patients referred to social work.
7. Educate patients and families, assisting with advanced directives.
8. Provide information and referral services regarding practical and environmental needs; serve as liaison with community agencies.
9. Develop and maintain contracts with public and private agencies as resources.
10. Participate in developing individualized care plans, involving patients and families, and contribute to interdisciplinary meetings.
11. Assist team members in understanding social and emotional factors affecting health and end-of-life issues.
12. Support bereavement programs, ensuring compliance and timely assessments post-death, including making condolence calls and organizing support groups.
13. Assist families with funeral planning, legal, financial, and healthcare decisions.
14. Ensure compliance with HIPAA regulations, maintaining confidentiality and minimum necessary use of protected health information.

Additional Duties

• Attend interdisciplinary meetings and participate in quality improvement activities.
• Engage in ongoing professional development through continuing education.
• Perform other duties as assigned.

Qualifications

• Bachelor's degree in social work from an accredited institution; Master's preferred.
• Licensed in the state of practice.
• At least one year of healthcare social work experience; hospice experience preferred.
• Strong communication and organizational skills.
• Current CPR certification.
• Ability to pass DHS background check.
• If driving is required: valid driver’s license, insured vehicle, and acceptable driving record.

Starting salary range: $60,000-$85,000 annually, based on location, education, certifications, experience, and skills. Compensation may include benefits such as health insurance, paid time off, and retirement plans.
