We are hiring for a Medical Social Worker.

Responsibilities
  • Assess social and emotional factors related to patient's illness to determine ability to cope with daily living problems as needed.
  • Assist the patient and significant others to understand, accept, and follow medical recommendations as needed.
  • Assist the patient and significant others in utilizing community resources as needed.
  • Identify gaps in community resources and stimulate resource development and/or improvement as needs arise.
  • Visit patients according to Plan of Care. Complete a clinical note for each visit and submit clinical notes to the agency daily.
  • Complete all required hospice modules in the online learning system and attend monthly in-services.
  • Function as Bereavement Coordinator and supervise bereavement services if needed.
  • Assist the healthcare team in understanding social, emotional, environmental, and social factors related to the patient's health status.
  • Be an alternate social worker for other agencies in the state when needed.
  • Contribute to developing a comprehensive, integrated Plan of Care for patients daily.
  • Instruct healthcare team members on community resources available to assist patients.
  • Collaborate with hospitals and community agencies to promote continuity of care.
  • Serve as a preceptor and mentor for new social workers, providing orientation and ongoing education.
  • Assist with Social Work and Bereavement Policy development and revisions.
  • Provide quarterly education to hospice staff on psychosocial issues.
  • Facilitate monthly caregiver support groups.
Education and Experience

License Requirements

  • Bachelor's Degree in social work from an accredited school or equivalent degree with supervised MSW.
  • One year of social work experience in a healthcare setting.
  • Current CPR certification.
  • Valid Driver's License, vehicle insurance, and access to reliable transportation.
