Job summary

LilyCare Hospice in New Jersey seeks a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. Based in Florham Park, NJ, you will assess psychosocial needs and support patients and families on your caseload.

You will provide counseling, coordinate with the care team, review eligibility for benefits, and connect families to community resources. The role may transition to full-time as needs grow.

Qualifications

• MSW from a CSWE-accredited program required.
• Current NJ LSW or LCSW license issued by the State Board.
• One year of social work experience in healthcare; hospice/geriatric preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Serve as Hospice Social Worker for patients and families on caseload and assess psychosocial needs.
• Provide counseling and support to patients, families and caregivers.
• Evaluate insurance coverage and connect families to benefits and community resources.
• Participate in IDG meetings and keep the team informed on psychosocial changes.
• Complete EMR documentation and support staff orientation.

Hospice Social Worker

LilyCare Hospice is a community-based hospice serving patients and families across New Jersey. We are hiring a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. We are looking for someone to join us part-time, with an opportunity to work full‑time as well.

Duties and Responsibilities

• Serve as the Hospice Social Worker for patients and families on your caseload — assess psychosocial needs and build the social work portion of the individualized plan of care
• Provide counseling and support to patients, families, and caregivers, including anticipatory grief and bereavement follow‑up
• Evaluate insurance coverage and financial need, and connect families to community resources and benefits programs
• Participate in IDG meetings and keep the interdisciplinary team current on psychosocial changes
• Complete timely, accurate EMR documentation and support volunteer and staff orientation

Compensation and Schedule

The Hospice Social Worker role pays $36 - $42/hour based on experience. Full‑time, Monday–Friday day schedule. Territory is northern New Jersey, based out of Florham Park, NJ.

Skills and Qualifications

• Master of Social Work (MSW) from a program accredited by the Council on Social Work Education
• Current New Jersey LSW or LCSW license issued by the State Board of Social Work Examiners
• One year of social work experience in a healthcare setting; hospice, home health, or geriatric experience preferred
• Valid driver's license and reliable transportation
