Job summary

LilyCare Hospice, serving eastern New Jersey, is hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up. The role covers a caseload in private homes and facilities with a Florham Park, NJ base.

You will conduct assessments, develop plans of care, and coordinate pain and symptom management with the attending physician and Clinical Director while educating and supporting patients, families, and caregivers.

Qualifications

• Current RN license in New Jersey or a compact state.
• One year of RN experience required; hospice, home health, or case management experience preferred.
• Current CPR certification.
• Valid driver's license.

Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Clinical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits

• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Clinical Director

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday-Friday day schedule with a rotating on-call assignment. Territory is based out of Florham Park, NJ.
