Job summary

Brookings Health System in Brookings, SD is seeking a part-time Social Worker to join our hospice team. You will perform psycho-social assessments, plan care, and provide interventions while coordinating with families and the hospital staff.

This role includes bereavement coordination, consultation with social services, and helping patients live safely at home. A BS/MSW is required; hospice experience is preferred. We offer strong benefits and a supportive, award-winning rural health system.

Will also maintain open communication and provide consultation among families, patients, tenants, social services designee, hospital discharge planner, and nursing staff. Responsible for performing assessments that pertain to suicidal ideation, abuse and neglect, safety, ability to live independently, and mental health. In addition, this position includes coordinating the hospice bereavement program, as well as serving as hospice volunteer coordinator.REQUIREMENTS:Holds a bachelor (or related field) or master's degree, at a minimum, in social work;Supportive of the Hospice concept and demonstrates understanding of the needs of the terminally ill person, family and significant othersHospice experience preferred
