Job summary

Brookings Health System in Brookings, SD is seeking a part-time Social Worker to join our hospice team. You will perform psycho-social assessments, plan care, and provide interventions while coordinating with families and the hospital staff.

Qualifications

• Requires a bachelor or master’s degree in social work.
• Hospice experience preferred.
• Performs psycho-social assessments and plan of care as part of hospice team.

Responsibilities

• Perform psycho-social assessments and plan of care.
• Provide psycho-social interventions and consultation among families, patients, social services designee, discharge planner, and nursing staff.
• Coordinate the hospice bereavement program and serve as hospice volunteer coordinator.
