This full-time role includes Monday-Friday, 8a-5p, and provides comprehensive psychosocial services to patients, families, and caregivers. Requirements include a Master of Social Work from an accredited program, a state social work license (LMSW/LCSW/CSWA/ACSW), a valid driver’s license, and at least one year of health care social work experience; hospice experience is
Qualifications
- MSW from a school of social work accredited by the CSWE.
- State license to practice as a social worker (LMSW/LCSW/CSWA/ACSW).
- Current Driver's License.
- Minimum 1 year of social work experience in health care; hospice/palliative care preferred.
Responsibilities
- Provide psychosocial services and supportive counseling for hospice and home health patients, families, and caregivers.
- Explain hospice philosophy, coverage, and payer source benefits to patients and families.
- Offer counseling on psychological aspects of illness, end‑of‑life care, and related concerns.
- Serve as liaison with community agencies to advocate resources for patients and families.
