Job summary

Brookings Health System in Brookings, SD, is seeking a part-time Hospice Social Worker to join our interdisciplinary team. The role focuses on psycho-social assessment, care planning, and patient-family support within hospice services.

The position supports communication across families, patients, and care teams, with duties including bereavement coordination and volunteer coordination. Requires a social work degree; hospice experience preferred.

Qualifications

• Holds a bachelor or master's degree in social work.
• Hospice concept understanding and sensitivity to terminally ill individuals and families.
• Hospice experience preferred.

Responsibilities

• Conduct psycho-social assessments and care planning as part of the hospice team.
• Provide psycho-social interventions and coordinate family communications.
• Assess for suicidal ideation, abuse/neglect, safety, independence and mental health.
• Coordinate bereavement program and serve as hospice volunteer coordinator.
