Job summary

Rochester Regional Health Home Care is seeking an Assistant Director of Community Support Services to oversee clinical services for designated programs in line with state and federal regulations and agency mission. You will collaborate with leadership to develop patient care programming and address organizational challenges.

The role requires strong management, regulatory knowledge, and travel across regions to ensure staffing and quality patient services.

Qualifications

• Bachelor’s degree in Nursing or Master’s degree in Social Work required.
• Minimum five years of health care experience; three in supervisory role.
• Valid NYS RN license or NYS LMSW.
• Valid NYS Driver's license required.

Responsibilities

• Oversees the administration of the social work, volunteer, bereavement, pastoral care and music therapy departments.
• Directs Care Transitions and Health Home programs.
• Identify opportunities for managers to develop greater skills in clinical specialties of home health, hospice and palliative care and in performing their managerial function including providing educational opportunities to address these needs.
• Responds to employee/patient complaints and attempts to resolve in a timely and effective manner in accordance with policy and procedure.
• Monitors and/or designates appropriate personnel to monitor corporate compliance requirements of the agency in accordance with all federal, state, and local regulations related to delivery of health care services and personnel.
• Directs all service coordination to ensure staffing patterns are consistent with patient needs for service and care. Ensures that a sufficient number of staff is available to direct and deliver services to patients and manages and evaluates service management personnel and coordinators.
• Ensures adequate staff education through direct oversight of orientation and in-service programs. Delegates responsibility for coordination and implementation of orientation and in-service programs to ensure personnel awareness of current agency policy and procedure. Encourages staff development through ongoing education.
• Maintains ongoing relationships with patients, staff, outside agencies, facilities and association representatives.
• Participates in process improvement activities and in the development of policies, procedures and guidelines.
• Participates in the On Call rotation.

• Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.
• Maintains knowledge of all relevant legislative and regulatory mandates and ensures that all activities are in compliance with these requirements.
• Conducts periodic staff meetings to include timely distribution and education related to departmental and Ethics/Compliance information.
• Regular and reliable attendance is expected and required.
• Performs other functions as assigned by management.
