Job summary

Rochester Regional Health in Rochester, NY is seeking a Team Leader in Clinical Services for Hospice and Palliative Care. You will lead a clinical team delivering direct Home Care services, supervise staff, and support program development.

The role requires a Bachelor’s degree aligned with licensure, NYS license in a related field, and at least one year in Home Care. Travel may be required; schedule is 40 hours/week, Sundays through Wednesdays, 8:30am-5pm.

Qualifications

• Bachelor's degree aligned with license.
• One year of experience in a professional Home Care setting.
• Valid NYS license in Social Work, Physical Therapy, Occupational Therapy or Speech Therapy.

Responsibilities

• Leads orientation and training for new staff and provides clinical guidance.
• Ensures timely referrals, guides case management, oversees service delivery and documentation standards.
• Develops in-service programs and staff education sessions.
• Adheres to agency standards, privacy regulations, and corporate ethics; represents the agency in community meetings.
• Conducts staff meetings and ensures attendance reliability.
