Job summary

Rochester Regional Health Home Care is seeking a Hospice Liaison to facilitate patient discharge from facility to certified hospice services. The role emphasizes exceptional customer service and alignment with the Brand Promise.

The ideal candidate is a NYS-licensed RN or a NYS Social Work license holder, with experience coordinating interdisciplinary teams and managing complex cases in home/hospice settings. The position is full-time, daytime/evening schedule, with potential 10-hour days.

Qualifications

• Nursing AS degree is required.
• Experience collaborating with an interdisciplinary health care team is preferred.
• Strong written and verbal communication skills are essential.

Responsibilities

• Evaluate eligibility, needs and care resources for each patient referred to RRH Hospice Agency.
• Obtain insurance information and authorizations for reimbursement.
• Complete a comprehensive clinical assessment and develop a hospice care plan.
• Obtain MD orders to support the hospice care plan.
• Coordinate services per the care plan (supplies, equipment, etc.).
• Document all required records and RRH Hospice Agency documentation.
• Collaborate with care coordinators, social workers, and physicians to solve complex cases.
• Represent RRH Hospice Agency in community groups and RRH committees.
• Participate in marketing efforts as assigned.
• Serve as a community resource educator.
