Job summary

Suncrest Hospice seeks a Director of Social Services to lead our social work team and ensure the highest standards of patient and family support. You will set goals, oversee daily operations, and align social services with CHAP standards and regulatory requirements.

In Illinois, you’ll recruit and develop staff, manage budgets, and partner with clinical services to coordinate care, while maintaining accurate documentation and driving strategic planning to address unmet needs.

Qualifications

• Master's degree in Social Work required.
• LCSW certification required.
• At least three years of management experience in home care operations.
• Ability to supervise professional and administrative staff.
• Strong communications and public relations skills.

Responsibilities

• Direct daily patient social service operations and oversight of social service staff.
• Ensure compliance with state, federal, and CHAP standards.
• Implement improved work methods to align with regulations.
• Coordinate care with clinical services to ensure seamless home care.
• Assist in annual budget preparation and resource allocation monitoring.
• Recruit, orient, and manage social service staff.
• Oversee assessment, planning, implementation, and evaluation of social service care.
• Ensure documentation of social services in clinical records.
