Job summary

UnitedHealth Group is seeking a Hospice Liaison/Hospice Sales Rep to drive referrals and census growth by partnering with physicians, hospitals, and community providers. The role educates partners on hospice services, facilitates timely referrals, and supports transitions with the care team.

The position requires cultivating relationships, on-site visits, CRM documentation, and achieving admissions and payer-mix goals.

Qualifications

• 1+ year of experience in a healthcare related business development/outreach role.
• Access to reliable transportation, current driver's license, and liability insurance.

Responsibilities

• Educates health care professionals, patients, and families about home care services
• Develops and maintains relationships with key referral sources to generate leads
• Coordinates patient admissions with the team, including required documentation for referral to admission
• Conducts on-site visits to assess patient needs and barriers to care
• Keeps accurate records of patient interactions and referrals
• Achieves admissions, product, and payor-mix goals
• Meets short and long-term target account goals
• Documents the minimum expectation of sales calls daily in CRM with pre/post notes
• Maintains a complete record of targeted referral sources in CRM territory
• Attends community events to promote Amedisys
• Responds to customer complaints in a timely manner
• Performs other duties as assigned
• This role will travel 50% or more within the territory
