Job summary

Suncrest Hospice is seeking a Registered Nurse (RN) to provide in-home and facility-based care as part of an interdisciplinary team. You will manage a patient caseload across homes, SNFs, and assisted living facilities, delivering clinical assessments, psychosocial support, and hospice services.

The role requires an active RN license, strong teamwork, and the ability to travel within the service area using your own vehicle.
