Job summary

The Pennant Group seeks a dedicated Hospice RN to join our West Phoenix team, delivering compassionate care to patients in their homes and communities. You will assess patients, design and adjust care plans, administer medications, and support families through difficult moments.

Join a collaborative team with ongoing training, a Monday–Friday schedule, and a focus on patient-centered care. A valid AZ RN license and driver’s license are required, with hospice/home health experience preferred.

Qualifications

• Currently licensed as a Registered Nurse (RN) in the State of Arizona.
• Minimum of 2 years of nursing experience; hospice or home health preferred.
• Homecare Homebase (HCHB) experience preferred, not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support to patients and families.
• Supervise hospice aides and delegate tasks appropriately.
