Job summary

Emblem Home Health & Hospice in the East Valley of Arizona is hiring an experienced Registered Nurse to join a patient‑centered home health team serving Chandler, Mesa, Gilbert and Queen Creek.

You will perform Start of Care visits, develop personalized care plans, and provide wound care and IV/infusion therapies as ordered, while coordinating with physicians and therapists to support patient independence and optimal outcomes.

Qualifications

• AZ RN license currently active.
• Minimum 1 year home health preferred.
• Wound care and/or infusion experience preferred.
• CPR certification current.
• Valid AZ driver’s license, auto insurance and reliable transportation.

Responsibilities

• Perform Start of Care (SOC) patient visits with initial assessment, medication reconciliation and home safety checks.
• Create personalized care plans to improve outcomes and support independence.
• Perform wound assessments and provide wound care as needed.
• Administer and monitor IV/infusion therapies per physician orders.
• Complete accurate and timely clinical OASIS documentation.
• Act as Case Manager, coordinating care with physicians, therapists and the interdisciplinary team.
• Educate and empower patients and caregivers with confidence.

Your Impact

Our Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Arizona
• Minimum of 1 year Home Health preferred
• Wound care and/or infusion experience preferred
• Strong clinical skills and a passion for patient-centered care
• Expert organizational and time management skills
• Current CPR certification
• Valid AZ driver’s license, proof of auto insurance, and reliable transportation
