Job summary

Emblem Hospice in the Phoenix area seeks a Licensed Social Worker (LSW) to assess psychosocial needs, provide counseling, and collaborate with an interdisciplinary team to ensure holistic, dignified end-of-life care.

You will support patients and families, participate in care planning, and have opportunities for growth through ongoing training. Travel within the greater Metropolitan Phoenix service territory is required.

Qualifications

  • Master's degree in Social Work from a CSWE-accredited program.
  • Must hold LMSW or LCSW as issued by the State of AZ.
  • Minimum 1 year in healthcare; hospice experience preferred.
  • Deep understanding of hospice philosophy and end-of-life care.
  • Current CPR certification.

Responsibilities

  • Comprehensive psychosocial assessments for patients and families.
  • Develop and implement care interventions based on evaluations.
  • Provide counseling and support to patients and caregivers.
  • Maintain timely clinical records for referred patients.
  • Coordinate referrals and liaise with community resources.
  • Participate in interdisciplinary care planning and meetings.
  • Advocate for social and emotional factors influencing care.
  • Contribute to quality improvement initiatives.

Requirements
