Job summary

James River Hospice in Richmond, VA is seeking a compassionate Hospice RN for weeknight on-call to provide after-hours nursing care and coordinate with the interdisciplinary team to ensure high-quality patient outcomes.

Responsibilities include after-hours nursing visits, initial assessments, admissions, and ongoing care planning with strong emphasis on family education and pain management. Requires Virginia RN license and hospice experience.

Qualifications

• Active Virginia RN license.
• Minimum of one year of hospice nursing experience.
• Graduate of an accredited nursing program.
• Valid driver's license and reliable transportation.
• Strong assessment and communication skills.
• Ability to work independently during overnight on-call hours.

Responsibilities

• Respond to after-hours patient calls and provide nursing visits as needed.
• Complete initial patient and family assessments to determine hospice needs.
• Perform hospice admissions as assigned.
• Develop and update individualized plans of care.
• Provide pain and symptom management.
• Monitor, document, and report changes in patient conditions.
• Collaborate with physicians and the interdisciplinary team.
• Obtain physician orders as needed.
• Coordinate care with LPNs and hospice aides.
• Provide compassionate support and education to patients and families.
• Complete required clinical documentation.

James River Hospice is actively seeking a compassionate and experienced Hospice RN Weeknight On-Call to join our growing team in Richmond, VA!

In this role, you will provide after-hours hospice nursing care, respond to patient needs, complete assessments and admissions as needed, and coordinate with our interdisciplinary team to ensure patients and families receive exceptional care.

Schedule: Monday - Thursday 5 PM- 8 AM

Salary Range: $85,000 - $105,000
