Job summary

James River Hospice in Lynchburg, VA is seeking a compassionate Hospice RN (On-Call) to manage patient care on an intermittent basis, coordinating with the interdisciplinary team for optimal outcomes. The role requires weekend on-call hours and travel to Lynchburg as needed.

Qualifications

• Registered Nurse with a current Virginia license.
• At least one year of hospice experience required.
• Graduate of an accredited nursing program.

Responsibilities

• Perform initial assessments of patients and families to determine hospice needs.
• Develop care plans based on nursing diagnosis and pallia­tive actions.
• Coordinate with clinical team and supervise LPNs and aids involved in patient care.
• Spend quality time with patients to support pain management and comfort.
• Modify and reevaluate the care plan as needed.
• Monitor, document, and report symptoms and changes in condition.
• Obtain physician orders as required.
• Participate in on-call duties per policy.
• Perform additional duties as assigned.

James River Hospice is actively seeking a compassionate and highly committed Hospice RN (On-Call) to join our growing team in Lynchburg, VA! You will be responsible for the hospice case management of your assigned patients on an intermittent basis in their place of residence. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!

Schedule: Friday at 5:00 PM through Monday at 8:00 AM. Must be willing to travel to Lynchburg as needed

Salary Range: $75,000 - $85,000

Territory: Lynchburg and surrounding counties

A brief overview of Responsibilities for Hospice Night Nurse:

• Complete an initial assessment of the patient and family to determine hospice needs
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Provides coordination amongst the clinical team supporting the patient, and supervising LPNs and Hospice aids who may be involved in supporting patients.
• Spend quality time with the patient to assist with pain management and support
• Modify and re-evaluate the plan of care when necessary
• Monitor, record, and report symptoms and changes in the patient's condition
• Obtain and receive physician orders as required
• Participates in on-call duties as defined by the on-call policy
• Other duties as assigned

Qualifications of Hospice Night Nurse:

• Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
• At least one year of hospice experience is required.
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available for use during work hours.
