Job summary

James River Hospice is seeking a compassionate Hospice RN Case Manager to oversee intermittent hospice case management in patients' homes. You will coordinate with the interdisciplinary team, supervise LPNs and aides, and ensure comfort and quality of life through hands-on pain management.

Salary Range: $72,000 - $82,000

Brief Overview of Responsibilities:

• The Hospice RN is responsible for completing an initial assessment of the patient and family to determine hospice needs and developing an individualized plan of care focused on comfort and quality of life. This role includes managing and coordinating care with the interdisciplinary team, supervising LPNs and hospice aides, providing hands-on support for pain and symptom management, and spending meaningful time with patients and families. The RN monitors, documents, and communicates changes in condition, obtains and carries out physician orders, adjusts the plan of care as needed, participates in on-call rotation per policy, and performs other duties as assigned.

Qualifications:

• Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
• At least one year of hospice experience is required.
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available for use during work hours.
