Job summary

Providence Hospice and Palliative Care in Homer Glen, IL is seeking a compassionate Hospice RN for a PRN, part-time role with weekend on-call responsibilities. Qualifications

• Active RN license in Illinois.
• At least 1 year of RN experience, preferably in Hospice or Home Health.
• Current CPR.
• Driver’s license in good standing.
• Reliable transportation to travel to patients' residences.

Responsibilities

• Provide skilled nursing care.
• Provide case management services.
• Conduct initial assessments of new patients.
• Travel to patients wherever they reside (home, skilled nursing facility, assisted living, etc.).
• Participate in an on-call schedule.
