Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) for a full-time day shift in Alaska. You will assess patient and family needs in the home setting, deliver professional nursing care, and collaborate with the Interdisciplinary Group to plan and direct individualized care plans.

Prior hospice/home health experience is preferred. Responsibilities include completing assessments, writing care plans, administering prescribed meds, and maintaining thorough documentation.

Qualifications

• Current, unrestricted RN license in the state of employment.
• Associate or Bachelor's degree in Nursing from an accredited program.
• Hospice or home health experience preferred and 1 year recommended.

Responsibilities

• Complete patient and family assessments to determine hospice needs.
• Provide comprehensive nursing care using the nursing process.
• Write and initiate Plan of Care with the IDG.
• Reevaluate needs and revise the Plan of Care as status changes.
• Administer medications as prescribed and participate in on-call shifts.
• Document patient status with accurate clinical notes and care plans.
• Assist in orientation and ongoing development of team members.
