Job summary

Moments Hospice is seeking a Hospice RN Clinical Manager/Team Director to lead an interdisciplinary care team, coordinating with nurses, social workers, chaplains, and volunteers. You will ensure regulatory compliance, oversee care plans, and drive quality improvement while supporting families and physicians.

This role emphasizes autonomy, collaboration, and leadership in end-of-life care, with an emphasis on seamless communications and a strong commitment to compassionate service.

Qualifications

• Registered Nurse (RN) license required with at least 3 years healthcare experience, hospice preferred.
• Bachelor's degree strongly preferred.
• Leadership/management experience is a major plus.
• Valid driver's license is required.

Responsibilities

• Lead an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings and document patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
