Job summary

Moments Hospice seeks a visionary RN Clinical Manager/Team Director to lead an interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers. You will ensure adherence to hospice standards, coordinate care with families and physicians, and drive policy development and emergency procedures.

This role offers growth through training, on-call support, and a central hub for seamless care coordination across the patient care continuum.

Qualifications

• RN license with minimum 3 years of healthcare experience, hospice preferred.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.
• Valid driver's license required.

Responsibilities

• Lead an interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings and document patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling and performance reviews, fostering a positive work environment.
• Act as central hub for communication with families, physicians, and providers.
• Analyze quality metrics and manage patient caseloads as needed.
• Participate in after-hours and on-call duties.
