Pathways Hospice in Houston, TX is seeking a Clinical Supervisor RN to lead quality hospice care, coordinate interdisciplinary teams, and ensure compliance with federal and state standards.

The role focuses on supervising care teams, evaluating patient plans, mentoring staff, and maintaining documentation accuracy. Strong nursing leadership and teaching skills are essential. Qualifications

• 2+ years experience as Hospice RN Supervisor.
• Supervisory experience in Hospice.
• Reliable commute to the Branch/Work out of Branch.
• RN licensed in Texas.
• Experience with HCHB EMR preferred.
• Knowledge of Home Health regulations and healthcare compliance.
• Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook).

Responsibilities

• Supervises, monitors, and ensures competencies and performance of assigned hospice care team(s).
• Oversees case management and ensures accurate evaluation and treatment plans.
• Reviews documentation for compliance and provides feedback on OASIS corrections.
• Participates in quality improvement activities and analyzes outcomes.
• Reviews patient schedules for coordinated care and efficiency.
• Educates and mentors the home health team on documentation and standards.
