Job summary

Hospice of Marion, a Part of Empath Health, seeks an RN (PRN) to join our On Call team for after-hours patient visits in homes, long-term care, and assisted living in Marion County. Day and overnight shifts, including two weekend 12-hour shifts per month, are available.

The RN will perform assessments, provide education and emotional support, supervise LPNs and Home Health Aides, and collaborate with the IDT to ensure quality Full Life Care.

Qualifications

  • Associate degree or equivalent required.
  • Minimum of one year related experience in acute or post-acute care.
  • Florida RN license; valid driver’s license.
  • Proficiency in documenting patient care per policy.

Responsibilities

  • Perform history and age-appropriate physical assessments; initiate referrals and collaborate with IDT.
  • Identify patient and caregiver learning needs and provide education.
  • Document in compliance with organizational standards and policies.
  • Provide end-of-life education to IDT and community members.
  • Supervise the care provided by LPNs and Home Health Aides; evaluate care every two weeks.
  • Assist with bereavement closure in collaboration with IDT and families.
  • Administer and monitor medications as prescribed by attending physician.

Hospice of Marion, a Part of Empath Health, is seeking an RN (PRN) to join our On Call team to visit patients and provide clinical, physical, educational and emotional support after-hours in the patient homes, long term care facilities and/or assisted living facilities in Marion County. Schedule: Day shifts and Overnight shifts are available; 2 weekend shifts per month (12-hour shifts) Since 1983, Hospice of Marion County has been honored to serve families across Ocala and Marion County with compassionate, expert end-of-life care. What You’ll Do

Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines. Identifies age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match. Documentation is compliant with all organizational standards including productivity, timeliness and adheres to relevant policies and practice guidelines. Following a patient’s death, assists with closure contact and support in collaboration with IDT. Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits. Educates and represents the community regarding hospice care philosophy and concept through a case by case consultation with persons in the community or educational inservice or seminar as assigned. Provides education to IDT members regarding nursing aspects of patient/family care. Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age-specific care practices and provide appropriate patient/family teaching.

What You’ll Need Education and/or Experience

Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of one year related experience and/or training; or equivalent combination of education and experience. Licensure: Valid Florida license as a Registered Nurse Valid Florida State Driver’s License Prefer one (1) year experience in an acute or post acute care setting Preferred experience caring for terminal patients and others with life threatening illnesses. For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
