Hospice of Marion County, a part of Empath Health, is seeking a compassionate and skilled LPN for our Crisis Care Team to provide exceptional support during critical moments of care. Providing one-on-one care in homes and facilities. Cross training in our inpatient unit and on-call visits. Shift: Night shift 8pm-8am, 36 hours a week; coverage area throughout Marion County.

Provide ongoing monitoring of physical, functional, psychological, social, and spiritual and safety needs of the Patient/Family/PCG and performs appropriate emergent skilled interventions, treatments, follow-through or referral in collaboration with Primary RN following the Plan of Care.
Provide skilled, competent nursing care under the direction of the Primary RN to crisis care team patients and their families.
Administer medications and treatments prescribed by the attending or hospice physician in collaboration with Primary RN and/or CTM and documents following Crisis Care Team MAR process and agency policy.
Provides comfortable, compassionate, dignified and supportive care prior to, during and following death with patient and primary caregiver following Empath Health policies and in collaboration with primary RN/Counselor and other IDT members.
Educate the community regarding Empath Health philosophy and represent Empath Health at nursing functions in the community as assigned by supervisor.
