Job summary

Empath Health’s Hospice of Marion County is seeking an RN (PRN) to join the On Call team, visiting patients in their homes and facilities after hours to provide clinical, educational, and emotional support in Marion County.

Evening and overnight shifts are available, with two weekend shifts per month (12-hour shifts). You’ll work with an interdisciplinary team to deliver compassionate Full Life Care.

Qualifications

• Requires RN licensure in Florida.
• Minimum of one year related experience and/or training, or equivalent.
• For field-based positions, reliable transportation with valid auto insurance is required.

Responsibilities

• Obtain patient history and perform age-specific physical, psychosocial, and spiritual assessments; initiate referrals and collaboration with IDT and physicians.
• Identify learning needs of patient and caregiver; tailor education and provide follow-up.
• Ensure documentation complies with organizational standards and policies.
• Provide after-hours care and support after patient death in collaboration with the IDT.
• Supervise and direct care by LPNs and Home Health Aides; evaluate services biweekly.
• Educate IDT members on nursing aspects of patient/family care and administer medications as prescribed.

Job description

Hospice of Marion, a Part of Empath Health, is seeking an RN (PRN) to join our On Call team to visit patients and provide clinical, physical, educational and emotional support after-hours in the patient homes, long term care facilities and/or assisted living facilities in Marion County.

Schedule: Evening and Overnight shifts available; 2 weekend shifts per month (12-hour shifts)

What You’ll Do

• Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
• Identifies age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match.
• Documentation is compliant with all organizational standards including productivity, timeliness and adheres to relevant policies and practice guidelines.
• Following a patient’s death, assists with closure contact and support in collaboration with IDT.
• Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
• Educates and represents the community regarding hospice care philosophy and concept through a case by case consultation with persons in the community or educational inservice or seminar as assigned.
• Provides education to IDT members regarding nursing aspects of patient/family care. Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age-specific care practices and provide appropriate patient/family teaching.

What You’ll Need

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of one year related experience and/or training; or equivalent combination of education and experience.
• Licensure:
  • Valid Florida license as a Registered Nurse
  • Valid Florida State Driver’s License
  • Prefer one (1) year experience in an acute or post acute care setting
  • Preferred experience caring for terminal patients and others with life threatening illnesses.
  • For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
