Job summary

Hospice of Marion County, part of Empath Health, seeks a compassionate LPN for our Crisis Care Team to provide one-on-one nursing in homes and facilities, with cross-training in the inpatient unit.

Shift is night 8pm-8am, 36 hours/week across Marion County; we offer competitive pay, comprehensive benefits, PTO, and opportunities for CEU support within a mission-driven team.

Qualifications

• Must hold a valid Florida LPN license and CPR certification.
• Driver’s license required for field-based, travel-capable duties.
• Experience in home health or hospice care is desirable.
• IV Therapy certification is preferred and must be obtained within six months of hire.

Responsibilities

• Provide ongoing monitoring of physical, functional, psychological, social, spiritual and safety needs of patients and families.
• Deliver skilled nursing care under the direction of the Primary RN.
• Administer medications and document per MAR and policy.
• Provide compassionate care before, during, and after death with the patient and caregiver.
• Educate the community about Empath Health philosophy and participate in nursing community events.

Job description

Hospice of Marion County, a part of Empath Health, is seeking a compassionate and skilled LPN for our Crisis Care Team to provide exceptional support during critical moments of care. Providing one-on-one care in homes and facilities. Cross training in our inpatient unit and on-call visits.

Shift: Night shift 8pm-8am, 36 hours a week; coverage area throughout Marion County.

What You’ll Do

• Provide ongoing monitoring of physical, functional, psychological, social, and spiritual and safety needs of the Patient/Family/PCG and performs appropriate emergent skilled interventions, treatments, follow-through or referral in collaboration with Primary RN following the Plan of Care.
• Provide skilled, competent nursing care under the direction of the Primary RN to crisis care team patients and their families.
• Administer medications and treatments prescribed by the attending or hospice physician in collaboration with Primary RN and/or CTM and documents following Crisis Care Team MAR process and agency policy.
• Provides comfortable, compassionate, dignified and supportive care prior to, during and following death with patient and primary caregiver following Empath Health policies and in collaboration with primary RN/Counselor and other IDT members.
• Educate the community regarding Empath Health philosophy and represent Empath Health at nursing functions in the community as assigned by supervisor.

What You’ll Need

• Licensure:
• Valid Florida LPN License.
• Valid Florida State Driver’s License.
• Certifications:
• Valid CPR certification from the American Red Cross or American Heart Association
• Current IV Therapy certification preferred. Must obtain the certification within six months of hire date.
• Additional experience in other health care settings desirable.
• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
