Job summary

Ohio’s Hospice in Newark, OH, is seeking a compassionate Registered Nurse (RN) for our Hospice Inpatient Unit on a PRN basis. The role covers the night shift (6:30 p.m.

– 7:00 a.m.) with every third weekend rotation, at the Hospice Inpatient Unit located at Licking Memorial Hospital. You’ll deliver patient-centered end-of-life care, collaborate with the IDT, and support families throughout the hospice journey.

Qualifications

• Graduate of an accredited RN program with an Ohio license.
• BSN preferred.
• Minimum of 2 years acute care nursing experience preferred.
• CHPN certification preferred.
• Current CPR certification.
• Strong computer skills and accurate EMR documentation.
• Valid driver’s license and safe driving record.
• Ability to travel as required.

Responsibilities

• Provide nursing care using the nursing process (assessment, planning, implementation, and evaluation).
• Assess patients’ physical, emotional, psychosocial, and spiritual needs.
• Develop individualized care plans with patients, families, and the interdisciplinary team.
• Review and update care plans, medications, interventions, and visit frequency as patient needs change.
• Monitor patient conditions and communicate significant changes to physicians and the interdisciplinary team.
• Participate in hospice admissions, interdisciplinary team (IDT) meetings, and care conferences.
• Document assessments, physician orders, interventions, and clinical updates in the electronic health record.
• Educate and provide guidance to caregivers, Personal Care Specialists, volunteers, and other members of the care team.
• Assist with onboarding and mentoring new clinical team members.
