Job summary

Qualifications

• Minimum 2 years of nursing experience including hospice or home health experience preferred.
• Valid driver's license and one year clean driving record.
• Good physical shape.

Responsibilities

• Establish a professional relationship with patients and families.
• Evaluate the patient's needs continually.
• Coordinate and revise the treatment plan.
• Render nursing care as ordered by the physician.
• Participate in Quality Assurance/Improvement processes.
• Prepare accurate and timely reports.
